This form is to be submitted by Facility Coordinators ONLY. USTA Colorado will allow facilities to force a mandatory reschedule for any home matches that are scheduled on submitted and approved blackout dates. Blackouts may only be submitted for the following reasons: Court construction, court maintenance, facility closure, or USTA Sanctioned Tournaments. Indoor PCT, club championships, socials, drills, or any other inhouse facility programming MAY NOT be submitted as a blackout. All blackout dates are tentative until approved by the league department.
