Please submit this form to confirm that your team is planning to participate in the Flight Playoff & District Championship as a 1st place team or wild card, should your team finish in 1st or draw a wild card.

Note: Some divisions may not have a Flight Playoff round. If your team is in a Flight Playoff, you must win the Flight Playoff round to be qualified to advance to Districts.
By submitting this form, teams are confirming their ability to participate in the Flight Playoff round and District Championships, should they qualify.
